Most haunting decisions - in my experience - always revolved around people. Whether it was not hiring someone early enough and missing out on a major opportunity, or even retaining them on the team by giving them better resources, pay, etc. Or worse, not firing/parting ways with someone soon enough before they caused disastrous damage to the business and culture.. Those types of people decisions linger around in one's head for a long time.
